Check / West Valley care notes

Check the person. Then the pharmacy.

You have a clinician’s name and the letters after it. Use the board for those letters, then make a separate pharmacy check.

Start with the right Arizona board

These board-search notes reflect the October 3, 2026 record.

Permission to practise

Search by the person’s role

Read beyond permission to work today

Match the full name and role to the person you are considering. Open any linked board decisions.

Look for penalties or limits on the person’s work. If a decision is hard to follow, ask the board to explain it.

A licence check does not rate how good someone’s care will be.

Under rules checked October 6, 2026, the board’s online list of penalties and other final decisions may leave out older records.

An empty list does not prove there were no earlier complaints or board decisions.

Ask that person’s licensing board for public records of past penalties or other final decisions. Some kinds of records cannot be released.

Ask for the pharmacy’s full name

The pharmacy that prepares a medicine may differ from the name on a care website.

Rules checked October 3, 2026 require an Arizona permit for a pharmacy that ships here from another state. It also needs authority where it operates.

A pharmacy permit alone does not prove that each made-to-order medicine meets the law’s requirements.

Ask for the permit details before comparing pharmacy claims.

For the pharmacy check

Next, check the medicine’s approval as well as the pharmacy’s permission to operate.
